New Client Checklist
- Confirm I am taking new clients.
- If I am, confirm I take your insurance (if you have insurance) by calling my billing company at 801-562-4484. If I do, give them your billing and insurance info over the phone.
- If I do take your insurance, or you are going to pay out of pocket, schedule a session by booking an appointment (https://app.acuityscheduling.com/schedule.php?owner=14429015).
- Please put in your number so you (or your parent/partner) will receive a text reminder of your session.
- Arrive on time

No show policy
I require a 24 hour warning of cancellation so I can offer that slot to another client who needs support. After a warning, you will be charged the full out of pocket rate for each additional no show/late cancellation.
