Thank you for choosing to work with us! If you are a new client, please follow this protocol BEFORE requesting an appointment:

  1. Confirm we are taking new clients (Court 801-651-3907)
  2. If we are, confirm we take your insurance
  3. If we do take your insurance, or you are going to pay out of pocket, schedule a session by booking an appointment
  4. Fill out all the information, be sure to notice the section for “insurance/self pay” and choose insurance so you can fill out your insurance information. Also especially notice the entering of your phone number to receive session reminders, the no show/late fee policy and the permission to charge credit cards.
  5. Arrive on time

For existing clients, please use the client portal to schedule sessions, make payments, and send secure messages.

No show policy

We require at least a 24 hour warning of cancellation so we can offer that slot to another client on the wait list. There is a $100 fee for a no show/no call, and a $50 fee for a late cancelation (the day of) This applies even with telehealth sessions.
